The Oregon Community Foundation has contracted with Oregon State University to work with Douglas ESD in evaluating the needs of families in your community. We are interested in hearing your perspective on your community’s support of young families and the services you would like to see offered in your community.
The survey should take you 15 minutes and you can exit the survey at any time. Information you provide on the following questions is voluntary and will remain confidential. Information you provide will be combined with information from others in your community and reported in summary form only.
For more information on this parenting education initiative or questions about local opportunities, contact Analicia Nicholson, Analicia.firstname.lastname@example.org, 541-440-4777.
